Frequently Asked Questions

What insurance/payment methods do you accept?

We accept most major insurance plans and self-pay options. You can pay through our patient portal using any major credit/debit card, as well as HSA/FSA. Your information is encrypted and processed securely. If you have any questions about your balance, we're happy to help.
 

How do I schedule a service?

We offer flexible scheduling to meet your needs. Appointments are available Monday through Sunday, by appointment only. Contact us via phone or email for current openings.
 

How do I complete/sign patient forms?

All necessary forms can be found and completed through our patient portal. Sign in to access forms, send messages to your provider, request prescription refills, view visit summaries and billing statements, and more.
 

Do you complete disability forms?

To ensure accurate and appropriate documentation, disability-related forms are generally considered only after a patient has been in ongoing treatment with the practice for at least one (1) year. Completion of disability forms is not guaranteed and is determined on a case-by-case basis based on clinical appropriateness.
